Concussion Awareness - Important Documents For Coaches

In addition to taking the Concussion Awareness training described previously, all AYSA coaches are required to carry the forms attached below with them each time they take the field with their teams.  This includes all games and practices.

The first form - Return To Play - What Coaches Need To Know - outlines what Ohio's 'Return To Play' law means for coaches.  It describes, in detail, what coaches should look for if they believe a player has sustained a concussion and outlines the protocol coaches should follow in the event that a player is injured.

The second form - Concussion Notification Form - is to be filled out each and every time a coach believes a player suffered a concussion.  The form is to be filled out by the coach, who will go over the contents with the parents of the injured player.  Both parties will sign the form after the meeting concludes.

Please take a moment and print out these forms and keep them in a safe place that is with you at all times on the sidelines.
